Student Athlete Forms

This page will inform you and your parent(s) of the requirements of Luther College, which must be on file prior to the start of your participation in athletics. The First Day of Practice Checklist will provide you with all information needed to make sure you have everything completed and turned in when you arrive on campus. All student athletes need to complete a pre-participation physical examination prior to the start of the athlete's season. Also required on file is information regarding insurance coverage, Sickle Cell Trait, emergency Contact, assumption of risk and ADHD (if applicable).

1. Athletic Training Forms - Mandatory Pre-Participation Paper Work - All individuals that will be participating in or trying out for a sport will be required to have their forms completed before they are allowed to tryout, practice or participate in team activities.

3. If you are transferring to Luther from another institution, your official transcript needs to be sent and received by the Luther Registrars Office prior to your first practice Yes Yes
4. If under 18 after August 1st, print out NCAA Drug Testing Consent Form and have parent/guardian sign and return to Hollie Gray III (Assistant to Compliance Director) at grayho01@luther.edu or mail it to him at the address listed below. Yes
5. If under 18 after August 1st, print out NCAA HIPPA Consent Form and have parent/guardian sign and return to Hollie Gray III (Assistant to Compliance Director) at grayho01@luther.edu or mail it to him at the address listed below. Yes
6. International student-athletes (first-year enrollees and transfers) who initially enroll full time at an NCAA Division III school must have their amateur status certified by the NCAA Eligibility Center.
