Athletes will NOT be allowed to participate in any team activities, including strength and conditioning sessions with the Athletic Performance department, until all information is completed and accepted by the Athletic Training Staff.
Luther College Sports Medicine uses Athletic Trainer System (ATS®) to manage each student athlete pre-participation forms and medical records.Â
All individuals who will be participating in or trying out for a Luther College varsity sport will be required to have their forms completed and uploaded to the portal prior to August 1st.
Any paperwork which is NOT FULLY completed and uploaded to the ATS athlete portal by August 1st will result in a $25 late charge fee on the student account.

For any late paperwork, athletes will need to allow 24 hours for the AT staff to process before being able to participate in any team activities.

Physical
- Luther College requires an annual athletic physical to be completed each year and it must be filled out on the Luther College Athletic Physical Form.
- Must be completed by MD, DO, ARNP, or PA. Other providers will not be accepted per NCAA regulations
- Exam needs to be performed after June 1st in order to cover the academic year
- Hard copies will NOT be accepted. All paperwork must be uploaded to the ATS Athlete Portal.
- If Needed: Physical and Sickle Cell trait testing can be completed simultaneously in Luther Clinic with charges going through you primary insurance.Â

Sickle Cell
- The NCAA mandates each athlete show proof of their Sickle Cell trait status.
- Starting in 1991, all newborns in the United States were tested at the time of birth. Please check newborn screening for the hemoglobinopathy results.
- If unsure of your newborn screening results, you can also check with your state health department where you were born. Click below for state health department contact information: Link to State Health Departments
- If the student-athlete can not provide lab results from newborn screening, the athlete must be re-tested per NCAA rules.
